My Grandma is in US on Visit Visa but she is supposed to go back Nepal after her Nepali passport expires. Will there be any issues on Nepal’s immigration side?

There likely will be, best to renew her passport from the Nepali embassy in the US.
They scan passport at US Emigration. If before that, the Airline will check for passport. It’s unlikely she can fly with an expired passport from US, let alone Nepal pugnu.
You know, it’s likely that the airline is not going to let her board the plane unless she has a valid unexpired passport. If renewing her passport is out of the question, at the very least, you should prepare a travel document from the Nepali Embassy.
Renew the passport from Nepalese embassy in US. With an expired passport, she won’t be able to take her return flight from US.
Commenting for better reach.
Even with expired passport one can travel back to own country, but you need a certain paper from embassy itself. Better to check with immigration or embassy.
You have to have a non-expired passport to fly. It does not matter if you are flying home. She won’t be able to get a boarding pass in the US. You’ll need to renew it.
You can contact Nepali Embassy which can provide you an official certificate saying she is Nepali and has right to enter Nepal.
Renew the passport. You can get a travel document from the embassy but it’s a pain. No one seems to understand what that is and expect a lot of delays at every stop. Specially if not a good English speaker. Would not recommend. From personal experience.
